Thanks for reaching out to Jotform Support. Let me show you how you can create your very first form.

1. On the My Form Page, click on Create Form
   
   
2. Select Start from scratch
3. Select the layout, and for this, let's choose Classic Form
   
4. Click Add Form Element to add the type of data you want to collect
5. Under Basic tab, you can elements such Full Name, Email, Address and etc
6. Just select the element of your choice and make necessary edit. Like changeing the label name.
7. Once done, you are now ready to publish the form. Click on Publish
8. Copy the Form URL and share it
