How to reset email alert template on form

I created my form using a template as an example and cannot get the template info off

My form is

and when I fill it out as a test, I get an email with the subject Sacramento CERT uniform order  

The issue is related to email notification settings. You can edit the email notification template, or simply delete it, and then add a fresh one. 

You will have a default template with the fresh email notification.
