Form fields are not showing in the notification email

    I have created a jotform with several questions for our retreat attendees to fill out.  When I do a test email to me, it leaves out the last two questions every single time.  I even tried to clone and do a new one, but it always leaves out the last two questions of "gold or silver" and "t-shirt size".  I have saved it and when I preview, I can see the two boxes, but when I send it to me as the link in my email, those are left off.

    I cloned your form and I wasn't able to replicate your issue. I did a test submission on the cloned form and the last 2 form fields appeared on the email notification. Here's a screenshot of the form:

    Is it possible that this issue is already fixed? If not, please try to delete the notification and then create a new email notification.
