I see you are having an issue with receiving your email notifications for your form uploads.
The reason you stopped getting email notifications from us is because your email address ended up in our bouncelist which appears to be due to complaints from your domain.
I have removed it for you so that the emails start to flow again for you and you can see the reason for this below:Result: firstname.lastname@example.org is IN the bounce list
Reason(s) : Complain about message from AmazonSES
If for some reason you have this problem again we have an email tool that you can check and remove your email address from our bouncelist if needed.
Also, please note that you can always monitor your outgoing emails to your email address by taking a look at your email history.
