Hello, we need a form with "Add" additional form function - customer can add as many forms (same form) as needed. See the following link for example, and you can click "Add Gene" button to get duplicate form: http://www.biomatik.com/custom-gene-synthesis-quote

    Thanks,

    Michael

Thank you for your interest in our service.

We do not have an option to dynamically generate another form field set, but something similar is possible with a Configurable List Widget. Please note that conditional logic is not supported by that widget. You may check the following guide to get the idea.

How to Set Up the Configurable List Widget

Another option which is probably more suitable in your case is to add all the fields to your form, and show them based on conditional logic (when the Add button is clicked).

Please check the next form example:

I have added the fields for up to 5 gene field sections, and added the show conditions.

If this is something that might work in your case we can provide you with further instructions.
