It looks like that the notification on the form is edited. You may add a new notification to the form so that the fields will be added automatically in the notification. You may delete the old notification from the form once you have the new notification setup correctly.

You don't see any field in the email because you didn't add any field in the email configuration. Please find the screenshot below to see:
