I copied a form and made some changes to it. But only the old form is showing up in the email responder template. How do I fix this?

Hello maryhm,
Please note that when you edit your form's notification email template, it becomes dirty and any changes you make in your form will not reflect in your email alert.
The best way to solve this problem is to delete the existing email alert and a new email alert. A new email template will always include all the questions of your form The following guides should help you:
