Cannot login account suspended

    It has been too long and my forms are no longer on my website. I'm trying to log on and it says my log on info is not accurate.  Please help.  My user name was Katoosue.

    Your account is suspended because you're phishing for credit card information. We will activate your account but we need to delete the guilty fields. Let us know if that's okay with you. 

    Also, once the account is activated please use our payment tools to avoid getting suspended again. Gathering credit card information is against our terms of use.

    Thank you for confirming to delete the guilty fields. I have now deleted the guilty fields and activated your account. You can now login to your account. Please use our payment tools to avoid getting suspended again.
