I changed email recipient but email template is not displayed correct.

    In my form, I changed who the form will be emailed to, and now the form has changed formatting, and I am not able to set it back the way it was.

    Please help.

    Hello fivetownhoopdreams,

    I'm sorry for the trouble caused to you. 

    Please note that changing "Recipient Email" of email alert will not cause the changes in the email template. I did check your form's "Revision History" and found that you have made several changes to form today. Please check the screenshot below:

    To solve this problem, I would suggest you to please delete the existing email alert and add a new one. The following guide should help you:
