You can insert the E-signature widget or Smooth Signature widget in your form. Here's a guide on How-to-Add-a-Widget-to-your-Form.

In the PDF submission, you can enable the "Show Headers and Texts" option in the Submissions page. In this way, the text elements and headings will appear in the PDF submission. Here's a guide: How-to-Show-Headers-and-Text-in-the-PDF.
