The form I made and what is emailed to me after the "submitted button is clicked is not the same form there are addtions that I dd not add
Please look at Cathryn Eagle submission which is correct and look at what I received (attached)
Also I'm seeing that there are 9 submissions at this writing. I can only see Eagles an my Test. If there are others they can be deleted

I would suggest you to do the following:
1. Delete the current email notification.
2. Add a new email notification and it should be updated with the current fields on your form.
Regarding the submissions, yes, it is possible to delete the submissions you have been testing with, this guide will help you doing it: https://www.jotform.com/help/377-How-to-Delete-Form-Submission-Data
