I have edited a previously created form, however, when a staff member completes and uploads the form it reverts to the submission of the old form version.

I'm confused with this line "it reverts to the submission of the old form version" - Were you perhaps referring to the email you receive after each submission? Meaning, instead of seeing the new fields you added/edited on the form, you're still seeing the previous form fields on the Email Notification template?
If that's the case, you have 2 choices to choose from to fix this:
1. Either delete the NOTIFICATION then just re-add a new one. Re-adding a new NOTIFICATION will add all the new fields by default.

2. Or, you can just manually edit the template itself.
