How do I retain all of the fields and data from a form when it generates a PDF in the email?  I want to be able to populate this and then send for signature, but none of the terms and conditions, nor signature is retained.

To include the Text Fields you have on your form, go to your Submisions Page > click the GEAR icon > then tick the SHOW HEADERS AND TEXTS checkbox.

Afterwards, kindly test it by doing an actual submission to your form. Check the email you will receive and see if the Text Field and Signature are there.

BTW, it's worth mentioning that you can also edit the PDF itself. To customize the PDF template, just head over to the Submissions Page.
