    Hi Bart,

    Thanks for reaching out to Jotform Support. It's not possible to display a cookie banner in the form, unfortunately. Additionally, tracking form submissions using Google Tag Manager, is currently possible only if the form is embedded to your website using Source code embed. We have a guide about that here.
