Based on your explanation, the issue seems to happen only when you have embedded your form, may you please share us the link to the webpage so we can further investigate it?
Regarding the testing credit card numbers, which error are you receiving?
I would recommend you to check that your integration has the test mode turned on:

The credit card integration is live, so using any invalid credit card number will give you the same blank page result.

I tested your form with test card, and since it is a live payment integration, I got this error:
Which is correct.
I don't have a way to test with the live card I'm afraid.

That error works as expected. The issue is when clicking the Go back and fix link, it just takes you "back" to blank screen. Or the area where the form is usually loaded is blank, nothing gets loaded.
The user needs to refresh the page, losing all of the information they typed in previously.

Ok, if the form is redirecting back to an empty section where the form should be then there is an error since at least the form should be displayed there.

I also checked your page and it seems like you embedded your form using the JavaScript code, please try using the iFrame code instead and see if that makes any difference: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

There is no error whatsoever when clicking the Go back and fix link, just a white page.
I'll try embedding as an iframe and see if that changes anything.
