How to create form where user can fill out and we receive the submission?

and I can receive the information and a flyer can be generated for the customer online based on some of the information they provided

This company has a form and the information goes to them and then a flyer is generated.

I am sure this is very easy based on how point and shoot your instructions are but I do not have time to learn all this...
