How can I capture an opt-in on my website & then embed a Jotform Quiz within the site such that the user only has to enter their name & email address one time & it gets integrated with my site / JotForm / Mail Chimp

You may pre-fill a JotForm form through the URL, no matter if it's embedded or if you're using its direct link, here is the guide: https://www.jotform.com/help/71-Prepopulating-Fields-to-Your-JotForm-via-URL-Parameters
If you need to capture partial information on a first form and then continue with another one, then this guide will help you passing data from one form to another: https://www.jotform.com/help/351-How-to-Automatically-Pass-Form-Data-to-Another-Form
You may also turn your form into a quiz with this guide: https://www.jotform.com/help/289-Turn-Your-Form-into-a-Quiz-or-Exam-
And you can also integrate your form with MailChimp, this guide will help you doing so: https://www.jotform.com/help/163-How-to-Integrate-with-MailChimp
Now, I'm not sure if we have an integration that sends the data to your site, you may browse on our current integrations and find if there is anything that may work for you: https://apps.jotform.com/category/integrations
You may also create a report with your form submissions and embed such report on your site, this guide will help you with that: https://www.jotform.com/help/326-How-to-Create-Form-Reports-in-JotForm
I hope this helps.
