Will any of your subscriptions allow me to give a customer URL? Also, where will my responses to this form go?

To remove the JotForm Branding that is shown at the bottom of the form, you have to upgrade your account to any of the paid plans.
All account can remove the JotForm from the form URL, by setting up custom domain. Inside of this guide, you can find how to do it:
You can also create a custom URL of your form:
The submitted data are stored on our servers and you can view them from your account.
You will also receive an email Notification to your email when your form is submitted.
