I would like to create a form exactly (or better) like the one in the link below.

JotForm is all new to me. Is it possible to create a form exactly, or better than the one from this website? SAMPLE.

I'm designing a website using WordPress and have been unable to find the plugin for this particular form. Seeing your forms I thought, if anyone can do this it's you.

I have checked the form on the website you shared with us and I can say that it is possible to create such form here in JotForm.

I noticed that the image at the top of the form changes as you select a card from the first field.

Thank you so much for your reply. The sample form example you provided was excellent. I am very happy to see it can be done. I also checked your "conditions" link. Very informative, however, I do have two additional questions concerning your forms:

1-Does Jot Form include an embed code, on any form I design, so I can directly add that form to the web page I am creating instead of creating an "Email-a-Friend" or "Sign-up HERE" link which would navigate the visitor to your "https://form.jotform....." form, for the visitor to fill-out. The example I provided originally resides on the church website.

I would also like to include the embed code to my MailChimp AD campaign. Would this also be possible?

2-Is this service free of charge? If not, how much would it be?

There are more and more features at JotForm. If you have any additional question, do not hesitate to ask.

We are always glad to check if the feature is present at the moment and if no, we can provide a workaround, of course, if possible ;)

The form created by Mike can be imported to your client's account. And from there, you can fiddle around with it, and inspect how the conditions were setup by my colleague. In my opinion, this is a great way to learn how a particular form was built.

Cloning a form was discussed in great detail on the article below. I know you prefer better directions rather than a general link but it's easier to send you the appropriate guides since they are way more detailed.

Here's the FORM URL https://jotform.com/80806269277971 that you will use when cloning the form.

Once you're done cloning the form, navigate to SETTINGS > CONDITIONS > and have a look at the conditions for that form.

Like Mike mentioned, you can conditionally show fields based on the user's input. You can also apply this approach to multiple fields if needed.
