Is there a way to use Jot Form for a Letter Writing Campaign.

I was wondering if I can use Jot Form to create a Letter writing campaign for our stakeholders to submit a letter to us to be forwarded to elected officials.  I want to use a form where the main body of the letter stays in the template and also shows up in response when I pull up the submissions.  The stakeholders would only need to fill out the date, their contact info and add any additional comments in addition to the pre-written letter.  Is that possible?

I want to use a form where the main body of the letter stays in the template and also shows up in response when I pull up the submissions.

I can see you have already added the text to your form, if you would like to keep that text and only include it in the submission, then you may enable the option the include the static text in the PDF report and include the report in the email notification, the guides below will help you doing so:

    https://www.jotform.com/help/305-How-to-Show-Headers-and-Text-in-the-PDF

    https://www.jotform.com/help/25-Setting-up-Email-Notifications

In email notifications you will need to manually add the text.

Now, if you need to keep that text and allow the users to type in some text boxes in the text then you may use the following widget: https://widgets.jotform.com/widget/passage_test

The text will be included in the submission and emails. This guide will help you adding it to your form: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form

You may then ask for other information through basic fields such as text box, text area or any other fields (more information about our fields here).

    I hope this helps.
