Hi there,
The form I've created is a staff evaluation form. The Managers would like to be able to add their own comments on each question after the staff member has completed it. To make the process paperless.
Alternatively when we download or view the completed form can we have Managers Comments boxes?

Hello Jen,
You could add some fields for the managers to leave comments. To do so, they would need to edit submissions.

If needed, you could email edit link to your managers: How-to-Let-Users-Update-Their-Form-Submissions-at-a-Later-Date
Also, it is possible to show comments fields only in edit mode: How-to-Show-or-Hide-Fields-on-Edit-URL
When you download submissions, submitted comments will be included.
