hi, is it possible to send a second form after the first form is submitted?

once the registration for, is submitted, can it be automated for the patient questionnaire to be sent to the same individual straight away?

I originally sent a query to you to see if it is possible to send a 2nd form to a person automatically after a first form has been filled in and submitted. You sent back this response. I am not sure it solves my question. I will clarify my questions:
I have a Doctor’s Office and patients can register online on our website.
I have a registration form (which is a short form with mainly essential personal information)

Thank you for clarifying on this request. We did not receive the full details of your request, however, based on what we received, am assuming both of your forms were created using JotForm. You can set up and Autoresponder Email containing the link to the second form.

The Autoresponder can be edited to include all the information you might need to present to the user informing them to fill the second form. It is also sent automatically once a form is filled. Be sure to add an Email field to your form to collect the user's Email to which the Autoresponder Email is sent.
