How to use a second form to append data to a prior submission?

    Hi there,

    I was unable to find any answer to this question in your knowledge base. We've set up a form to see if someone is eligible for our programs.

    We then take them to a thank you page that shows them if they are eligible for any programs.

    If they ARE, we'd like to give them the option to then say "Yes I'd like you to contact me about this/these program(s)." and ideally do it with a single click, which would append to their prior submission and show us, in one place, what programs they are eligible for, and if it is ok to follow up with them.


    Can this be done?

    Passing one form's data to another is possible with Jotform. However, there is no option you can automatically merge the data from two forms into one single place. You'll have to access each submissions page individually and compare data.

    I would suggest qualifying users within the form, and you can do this with conditional logic. Let say, add a Text field that would say they qualify or not and show that based on their selections in the form. You can add two Text fields, one to show when they qualify and another one when they don't qualify.

    The only downside of this method, maybe, is that people can choose some options again - trick you so they will qualify.

    I'm sorry, I think the best option would be to configure to change the thank you message based on a combination of inputs/selections in the form.

    Along with setting this up, set the unique question in the form so people cannot re-submit if they don't qualify.
