How do I set up a registration form for a workshop and keep track of who signed up

I want to create a website registration form for a one day workshop. We use Paypal. We want to integrate this form into our Weebly organization website.How do we do that?

How can we keep track of who signs up?

Will the person who created the form be notified or can a person other than the form creator be designated as the  tracker?
