Hi
I just recently created a form.
I did a test drive and find that my recipients are receiving the email from the sample as well as the following message:
How do I correct this?
Payment must be made once your registration is submitted or you will forfeit your spot. Payment can be made by e-transfer to treasurer@gheng.ca or you can pay by dropping off your cash or cheque to the GHENG Community room on: Sunday April 14 from 11am-1pm OR Monday April 15 from 6-7pm

You can edit and customize your email template as shown on this guide: https://www.jotform.com/help/430-How-to-Edit-the-Email-Template-for-Notifications-and-Autoresponders
As well, you can delete it, and create a new one if you want to use the default template:
