Looking into HIPAA compliant form service

    Hello,

     

    We are a small pharmacy, without any coding knowledge, and are looking to create a HIPAA compliant form or series of forms to collect patient health outcome data from both patients and providers. We have an ideal workflow, I am wondering if JotForm's Silver or Gold plans would be able to accomplish this. If so, is there anything I should know about how to accomplish it before making the decision to sign up for an account?

     

    Here is the workflow: 

     

    We are a pharmacy and want to establish an agreement with a medical practice or individual prescriber allowing us to share and use information from their patients on treatment outcomes. Once this agreement is signed by both parties we would like that practitioner's info to be stored in a way that can be referenced by other forms, we will call these approved practitioner's offices [MD]
     
    Then we want to have an informed consent agreement signed with the individual patients that the offices will share information about. These informed consent agreement once signed would be automatically approved, and the patient's uniquely identifying info would be added in some way to a list of approved values able to be references by other forms. We can call this[PT]
     
    Once a patient has an informed consent agreement with us we would like to build a form to collect info on their treatment outcomes. We would like this form to verify against the [PT]values that they have an established informed consent agreement with us. We would also like an optional drop-down box to show them available practitioners to associate their data with populated with the [MD] values
     
    Once a practice has an agreement with us regarding sharing of patient data we would like to have a form they can use to submit patient data to us. We would like any data they submit to be checked against [PT] values, but without the whole list of possible [PT] values being visible to them. 
     
    We would like to be able to control how we can receive responses to these forms. For instance, we would like patients to receive some sort of automatically generated unique identifier, maybe a 6 digit number. We would like for the patients to know this number and for any data being viewed by most of our staff to have this number in place of name, date of birth, etc. 
     
    We may want the medical practices associated with the patients or IT Staff at our facility to see non-anonymized patient info. Maybe the data could feed into multiple different google sheets with access control settings for each one?

    To get the benefits of the HIPAA compliance you may upgrade to either silver or gold plan, no matter which one you chose you will have the HIPAA compliance. 

    Regarding the features you're looking for, I think your requirements are very specific so although it's possible to somehow emulate your workflow, I'm afraid that will not be possible to exactly meet your needs. 

    For example, the part bout collecting some data with one form which should be available for other forms, I'm afraid that this will not be possible as forms cannot be connected on this way. 

    What you may do is to use our spreadsheet to form widget, this way you may download the data from one form and have it available on the rest of forms, but note that this process needs to be manually updated, this means that you will need to constantly download the data from a form and upload it on the other forms you need to use this data.
