The Form fields and the upload elements do not appear in the PDF template

    Moreover, I am unable to get uploaded images from a filled out form to display in any PDF template. And sometimes even images from a provided URL submission on the form do not show on any template I create.

    I believe you are referring to this Form:

    Since you have used a custom PDF  so you have to customize the PDF by dragging the Form fields to the right side.

    Hi. Thanks for replying. I had submitted a few inquiries on issues I was
    trying to resolve and I am really happy you guys have replied. However,
    because some time has elapsed and your short messages come without the
    original question, I'm having trouble understanding what your referencing.
    Without the original question, the answer is almost useless. How can I get
    the entire conversation to better understand the issue being addressed?
