Why is my account suspended?

    Could you please help me?

    My account is being suspended too. I don’t know why.

    I did check your account and found that is currently suspended because of the passport filed you have added in form "Deelnemersovereenkomst - Physical Therapy Abroad". Please check the screenshot below:


    You may continue to collect that information, however it is a must to add an  agreement displayed on the form and a signature, widget fields. The following guide should help you how to add widgets to your forms: https://www.jotform.com/help/252-How-to-Add-a-Widget-to-Your-Form

    Thank you for your response.

    I checked the links and information you sent me. 

    In my form I do have a widget for a signature and also placed a required checkbox in which our volunteers have to agree with our Terms and Conditions (Algemene Voorwaarden in Dutch, it links to our website with the online terms and conditions).

    I made this form with a laywer to be sure to follow the EU and Dutch rules. 

    Can you help me to check whether my form is ok now and we follow the rules?

    Because my ip is still blocked. (I thought that my account being suspended and my ID blocked was because of the same reason).

    Your current IP address is not blocked. What is your IP address which is blocked? Please click on the following URL to know your current IP address: https://www.whatismyip.com/

    Seems the IP address is blocked. I have whitelisted the IP address now.
