I am producing a really long form that has several repeating question fields that are broken up into categories. For example, this section of the form is for education experience and in that section there is a text box question that says "Hands-on workshop experience". Next to this question is where the person can fill out how many credits they've earned and where and when for the questions. After filling out and submitting the form, the results are emailed to us and we only see the field they filled in but not which section or for what question it is from.
How can I receive the full form that someone submitted with all the text and questions? Almost like a PDF copy of the form.

Thanks for reaching out to us. Please try disabling the Hide Empty Fields button on your form and let us know if you can see the text fields on the submissions.

Hello!
It still doesn't show the full form in the email that's sent. The email looks like this image below:
You see where it says CEU Points and When & Where? These fields are associated with a question text field that I am not seeing in the emailed results. How do I get those text fields to show in the emailed results? See image below of the PDF download:

Hi,
You have to include Text FIELDS header on your email templates(both for NOTIFICATIONS And AUTORESPONDERS) manually.
