You have created a lovely template for booking appointments, but I would like the submissions to go to a pdf/spreadsheet, so that each days appointments are in one place; rather than a load of emails. How can I do this?

    Thank you for reaching support,

By using our PDF Editor, you can customize your forms so that after a submission is made, it will send a PDF document.

You can also integrate your form into a Google Spreadsheet. Here is a Related Guide: https://www.jotform.com/help/228-How-to-Integrate-Forms-with-Google-Sheets

    I hope that helps.

    Best.

Thanks for the info. I tried integrating a Google sheet, but I get individual pages for each submission!

What I want to achieve is one page for each date, with the  names of submitters listed under the time slot they have booked.

This is normal practice for say a dentist, so that the receptionist can see all the patients coming on that day and at what time. That is what I am after.

I am not sure if I understand your query correctly. Please note that submissions sent to the Google Sheet shall be displayed in each row on the same sheet and the sheet shall be updated as the new submissions received.

Do you mean to create separate sheet in the spreadsheet for each submission received? If so, unfortunately, it is not possible with the current integration. You may export the submission data from the Google spreadsheet to Excel and run an offline report as per your requirement.

Let us know if you are referring something different. We will be happy to help.

I think it may pdfs I need. I have tried to create a dummy file. the time slots would be in the boxes and Names of submitees either beside or under each time slot.

Generate 1 page for each day.

I am going to need this information in this format to keep for my records of who has had which slot. This is going to be a Covid-19 requirement to start Bowling.
