My account has been suspended wrongfully

I am working on a HOYTS cinema project for year 12 Information Process Technology at Knox Grammar School whereby we have to make a wix website and then make forms to embed in the site. I have been suspended from jotforms by the 'anti-phishing' program for making the log-on section to the project whereby I ask for a username and password e.t.c. Could you please reactivate my account as this project is due in 2 days and none of my forms are now working. They are simply being created to be viewed and marked by my teacher. Please could I ask for the account to be reactivated asap.

I would like to inform you that, your account has been suspended for requesting Login info in your HOYTS Log-In/Sign Up form. Due to our Terms of Use, the gathering of certain private information such as Driver's License information, Credit Card details, Login Information, Social Security Information etc. is not allowed without the mandatory widgets.

You need to include E-Signature widget in your form. However, we also require that you include a Terms and Conditions widget with your terms for your users to read.

I went ahead and removed the offending fields from your form and reactivated your account. You are more than welcome to revert the changes in your form so the fields are restored and kindly add the necessary widgets so you do not get suspended again.
