How to create form for screening employees?

I am a home care provider and need to screen our employees prior to each shift. I would like to know how i could use your forms to complete this task

Prior to each employee's shift, we call them to check their temperature and ask 4 other screening questions.   Is it possible to create a mobile form in which the employee can either take a photo of his/her thermometer reading or input the info and answer the other questions. the answers would need to be sent back to the office/administrator.

in addition, can this be set on repeat as to not create a new form each day?
