Reactivate suspended forms

    Hello,

    I am reaching out with urgency to reenable my forms. I was informed they have been suspended due to suspected phishing. Not sure form the reasons, but my business is dependent on these forms to active. This is very frustrating to have to wait so long to investigate. I appreciate your efforts in helping get this issue rectified. Thanks.

    I did check your account "ReLivePT" and did not find any issue with that. Your account is already active and all your forms seems to work as expected.

    Have you received any email that that your account is suspended and forms disabled? On which email address you have received this email?

    Please let us know and we will take a look.

    Hello,

    My forms are still not active and I receive the IP banned notice upon attempting to direct to any JotForm url. See pictures.

    I did check your account and found that it is active. You seem to have 5 forms in your account and all seems to load correctly. Please check the screenshot below:

    What is your current IP address from where you are trying to access jotform site and see this IP block message? Please click on the following URL to know your current IP address: https://www.whatismyip.com/

    Hi,

    I filled out the phishing form, previously, like 5pm CST. This is almost 12 hours later and still an issue. Please find in YOUR system the request to have my forms and IP address whitelisted. My IP should have been submitted along the form. As you can see from the screenshots, they are not within the form builder. I would advise you to try accessing my forms through the URLs, not within the form builder. I've tried using a few IP addresses with same result... FORM NOT FOUND.

    I am sorry for the trouble caused to you. I did check your account and found that though it is active but one of your form "ReLive PT Patient Registration Form" has triggered our anti-phishing filter. 

    It seems you have added ID upload field without any required widget. You may collect ID upload, but you need to add an agreement displayed on the form and a signature, widget fields.
