Hello,
I am attempting to build an application using tables in a "paragraph" that will need information filled in. I don't see anywhere that I can change the properties to lock some cells and allow for filling in.
I need to know the best way to build the form as close to the pdf provided. This is time sensitive as the time period to put this out to the pubic is only about a week away.

There are multiple pages to our form and several tables. The person wanting this form would like to be able to have the user input their answers in the second column and attach or upload documentation in the third column. Some tables may have a space for a signature that we would like to use e-signature.

I would like to understand why fields I have built keep disappearing as well. On the first page, there is a field that was built in Paragraph, but now all of that content is gone.

I would like to insert the file upload widget and/or signature widget into the table I built, is that possible?

Hi Margaret,
The paragraph field does not allow for data input of any type. An input table could be used to capture text based input but we do not have a field that would allow for using widgets like the signature field to be used in the table.
Individual fields could be used and then those fields could be placed in the PDF output where you would like:
This would allow for producing the PDF output with data in the desired locations, but there wouldn't be any field that would allow for creating a table in the web form with the desired inputs that matches the table in your PDF.

Is there a template that comes close to the information we are asking for or do I need to build this long application from scratch?

Hi Margaret,
As it is a long form with variations within you would need to build the form from scratch - I can't see a template that wouldn't involve nearly the same amount of work to adapt. Alternatively, you might find it easier to convert your word doc to PDF and then build a form from that, more information available here.

The original form is converted from a pdf but the required fill ins weren't working.

Making the web form look and function exactly like the PDF copy likely would not be possible. The same PDF can be used, it would just require using individual fields in the web version of the form and placing the output in correct location the PDF.
That would allow for entering the data in the web version of the form in whatever type of field works best for that specific input, then outputting the data to the original PDF when the form is submitted.
