How to distinguish the email notification when sent based on a condition?

    We created a Jotform to send email notifications based on conditional logic. However, when we receive email notification we cannot tell which submission it is and when we click on the link to open in Jotform from the email, the specific form is not opened. So the email notification based on conditional logic does not inform us of anything.

    As I have checked your form, it seems that there's no email notification created in it:

    Is it removed? If you are referring to a different form, please provide us with the form's direct link? For reference: Where-to-find-my-form-URL

    Hello,
    The email notification is enabled in conditional logic and is active. We have tested it and do receive emails for ones we configured, however, the email has no information that helps us identify which submission it is.

    I'm not sure why it sends you the email, but the logic needs an email template before you can configure it. Please create one to avoid any problem. From the looks of it, you seem to have configured the conditional logic while the template still exists, and delete it after.

    As for distinguishing the email, you have to add the name of the email template in the body or subject line. If you wish the form title to appear, that is also possible.

    Hello,
    We do not want to receive email notifications for every form submitted. We only want to see forms where patients indicated that they were positive for COVID-19 because they require further action.
    The issue is identifying who submitted the form. When we receive the email, all the information is secured and hidden.

    We apologize for missing your point.

    We do not display the entry of a secure/PHI field in the email. You'll get a message that says "Hidden or Removed For Privacy".

    You have to view the tables page to see the data. If you wish these fields to appear in the email, you would need to disable PHI. Please note that we do not recommend disabling PHI, especially if it violates the HIPAA rule.
