Hi Bailey, thank you for reaching us.
We have rolled a new feature that will allow you to use Conditional Logic on your appointment widget. It works the same way as to how you would use Conditional Logic. You will simply add an Update/Calculate Field and choose your first Appointment widget.
Then, kindly add the following formula. You may customize the days as you see needed.
Afterward, please choose the second appointment for Summary and choose Start Date or End Date for Property.
Please give it a try and let us know how it goes.

do not see a second appointment created in jotform. Is that what it should
do for me?

Hi clinicphguymon, thank you for reaching back to us.
I have tested in on my end and was unable to recreate the issue. The second appointment appears on my end. To confirm, have you add two calendar events for two different appointments in your integration?
If you haven't, kindly give it a try and let us know how it goes.

clinicphguymonReplied on March 19, 2021 at 12:44 PMI am unable to get this to work.I think an easier solution might be just
to duplicate the submission and schedule them manually. Is there a way to
duplicate a submission exactly as it is?Also, after daylight savings our appointments seem to be all off...
sometimes they are scheduling at the correct time and sometimes they are
scheduling at 1 hour later. Can you advise me how to fix this? So far I am
not finding any rhyme or reason to when it is correct and when it is not.

Hi,
You can add the two appointment fields to your form, You can then apply a condition to automatically set the appointment of the second dose field 28 days later.
Here's the condition:
Here's a demo form I have created for you: https://form.jotform.com/210777501928056
I have moved your other questions to a new ticket so we can address them separately to avoid confusion. We will be responding via these threads shortly:
