    I do have additional questions about how to incorporate HIPAA-compliant JotForm features into my medical practice work flows. In particular, I'm looking to create a HIPAA-compliant form that asks questions about a patient's symptoms and condition, and then populates these answers into appropriate fields in a Word template. I have several additional related questions. Is there an option to chat with someone by phone about these questions in addition to this forum?

    Once you have created your account and upgraded to HIPAA compliance, all your forms would be under HIPAA.

    However, we cannot populate the submission data into a Word template.

    It is possible however, to attach a PDF document of the submission data.

    Thanks for the input. Why can't the submission data be populated into a Word template? Is that a technical challenge or is it due to HIPAA? Can the submission data be populated into some other type of non-pdf format such as csv? I want to be able to manipulate the data after submission. Can I still manipulate the data from a pdf?

    Hi,

    Currently, we don't have a direct integration with Word Documents. However you can try to use Integromat  to connect JotForm and a Word Template.

    Can the submission data be populated into some other type of non-pdf format such as csv?

    You may integrate your form with Google Sheet and download it as CSV.

    You may also directly download your submissions as CSV.

    I want to be able to manipulate the data after submission. Can I still manipulate the data from a pdf?

    You can edit your submission. If you download the PDF version of the submission, it will have the updated data.
