    How should I make conditions that unique response but can make more different response by a single responder....it means how can a person make more response on behalf of others but each response must be a unique one...should not make repeatative response..
    .please guide me

    Apologies, however, I am quite confused with your requirements.

    Please share specific example of what kind of data should be unique, age, address, notes?

    Please share the URL of your current form so we can check its setup, then let us know which part of it can be repeated and which part should be unique.
