How to send edit link to patient?

Can this approval flow be used to send patients a prefilled forms to complete and have reminder emails sent until the forms are done?

I have a form for new patients to fill before their first appointment:

I would like to prefill 4 fields: appointment date, patient email, patient name and visit reason. Then send this to the patient to complete and sign. I understand how to do this already just by making a prefill and emailing the link to them, but I'm wondering if this can be done through the approval workflow?
The reason I would like it through Jotform Approvals is setting that reminder email every X days so the pt can be reminded to complete the form. And also to have an expiration date for the workflow. And in the future, I would like to add conditions that send additional forms based on the "visit reason."
The workflow would need to have a different approver each time - bc of each time the form would be "approved" (meaning, completed) by the patient. Is this possible?

Hello Peggy,
Please note that approval flow can be triggered only after the form is submitted. In that case, the option to populate the field and sent to the patient cannot be trigger through approval flow.
You can populate the form fields of the initial form and send the URL to your patient. When the form is submitted by the patient, the approval flow will be triggered. Does this flow of form fits into your requirement?

No that won't help.
What if I "submit" the form myself and trigger the Approval Workflow for the patient to receive an email to edit the submission? As in, can they edit the submission if they click the "approve" button?

Hello,
You can add the edit link to your Autoresponder email.
Here's how to do it:
- Go to SETTINGS > EMAILS
- Click on Autoresponder email
- Click + Form Fields
- Click Edit Link
- Click SAVE
