Create Survery & Reports Inquiry

Good morning! I am a coach specializing in business, career, and personal growth.

I've created a 40 question survey.

The 40 questions are divided into 8 categories (5 questions per category).

Each question has 3 multiple options. Each of the multiple choice options has a numeric scoring value (2, 1, or 0).

I'd like to be able to send my clients a link to the survey/assessment anytime during the year.

I want to be able to tabulate their individual score. Manual is OK, but automated is preferred.

Then I want to produce a report with graphs/charts to display each client's unique results.

Can jotform help me any or all of this?

Thank you for reaching out to Jotform Support. We have survey form templates, you can check it out here to see if it suits to your liking. But you can achieve what you want to do with Jotform. We also have a report for every form so you can see the report with graphs/charts about your survey form.

Thank you Justin, your response answered my "general" question about whether or not jotform can do surveys with results, and from your reply I can see that yes, jot form can do that. Great! Thank you! 

But if possible I'd still like to know if someone there can answer my "specific" questions that will determine if my particular project is a fit with jotform. Specifically: 

1. Can I divide my 40 survey questions into 8 categories? (5 questions per category). If so, how is that accomplished with jotform?

2. If I offer 3 multiple choice options per question, can I score each response with a value such as 2 points for answer A, 1 point for answer B, and zero points for answer C? 

3. Can I send my clients a link to the survey/assessment that they can they can respond to anytime during the year? In other words, can the survey only be offered in mass or can it be offered individually? 

4. Can I tabulate a client's individual survey/assessment scores? (For example Joe answered 40 questions and scored 8 points in category A, 4 points in category B, and 10 points in category C, etc)? 

5. Or does jot form only provide combined aggregate data on surveys? (50 people responded to your survey and on average they scored an average of 57 points overall)? 

6. Is results tabulation manual? Or is it automated? 

7. Are the reports with graphs and charts create manually? Or are they automated?

You can accomplish this using Form Elements like Heading and Single Choice elements. Heading can be used to set the categories and Single Choice can be used for the questions inside the categories. For a quick overview of Form Elements, check out this guide.
