Questions before recommending your service

Thank you for reaching out! Regarding HIPAA, it's included, so no additionnal fees, just one button to toggle on or off. The only thing to keep in mind its that it's applied to the entire account and not on a form basis.

You can easily build a quizz form and share it as you wish, should anyone want to integrate it into their own website or other, we have a ton of sharing options, and if only the link to the form is needed, they can just share the link with their clients and/or prospects.

And should you or any of your friends and colleagues would like to make a general template that anyone can use,  turning your form into a Template is as easy as this:

• On your My Forms page, select the form you want to use as a Template, click on the More dropdown arrow, then choose the Share as Template option under the Publish panel.

• Input your desired Description of your Template, then click on the Share button.

Keep in mind that the Template you created would be shared with the Public for anyone to use. You would be able to access your template on the Form Template page. You can learn more about this from your guide on Sharing a Form in the Form Templates Gallery.

If you want a Template for Private use only, unfortunately, this is currently not possible. But, you could Clone your Existing Form instead. This way you can use the format of your other form for your new one.
