The two recipients in the form are not bounced listed. I think the issue is caused by your form email settings, specifically the custom sender email. You are using Gmail, I would suggest switching it back to noreply@jotform.com. Gmail has updated their own DMARC policy and they don't support this method anymore.
You can also use the SMTP method instead: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form.

Looking more at the described issue it seems there might possibly be some confusion perhaps.
Are you not receiving all of the information being filled in with your form and email notification or just not receiving email notification at all when your form has been submitted to you?
Can you please confirm or clarify that for us? I am asking because of what you are showing above.
Additionally, according to your email history our servers are sending out the email notifications.
Have you tried checking your spam folder to see if they end up there by accident?

max123jotReplied on August 8, 2016 at 7:46 PMit is possible i do not understand the flow of data
- jotform sends me the data user entered - nice
- quiz app shows the user info as desired, and provides an excell download,
but i have not seen this info sent primarily to my form
perhaps, i misunderstood, that the quiz info would mimic the jotform info,
but also include correct/incorrect ans and % score
thanks

I have tried setting up a quiz form to send the results to different email addresses and was able to receive them without any issue. The submissions can be exported to Excel from the form submission view.
I assume that you have now understood that only the notification email contains the correct/incorrect answer along with the percentage score and not the actual form submission data.
If you are referring to something different, please let us know. We will be happy to assist.
