Why was our account PQSYS flagged for phishing?

    We created a form off of a jotform template in order to have a secure form for clients to enter credit card information for purchasing product.  We are not interested in collecting credit card information but to allow them to send one time information for purchasing.  We have not published the form was only trying to preview it when our account was suspended.  How do we resolve this?

    Upon checking your account, the form titled "Software One Time Payment Authorization" is asking for credit card information from your users using plain form fields like text boxes. Please note that we do not allow this kind of information to be stored in our servers. This is against our Terms of Use and this activity is considered phishing in our end.

    I went ahead and deleted the guilty fields in your form and re-activated your account. Please refrain from creating similar fields/forms to avoid being suspended again, continuous violation on our terms will only lead to account suspension or permanent ban.
