I checked your form on an iPhone 6s emulator and it is showing up without any issue.
Is your form embedded on a website? If so, can you please give us the link to that website so we can check the form there using the iPhone emulator.
Please also try clearing the browser cache and load the form again.

It does not show on my iphone7 plus. I have to have someone else check their android device. A friend couldn't load it on her iphone6s. Here is the URL https://www.sstte.org/fundraiser-pay-online.html
i used your standard embed code for the website not the customized one for weebly. Would that make a difference?

I have checked your form on an Android and iPhone 6. I used Chrome for both of them. I replicated you issue. Please have a look at the following screenshots below:
My suggestion is to use iFrame code while embedding to Weebly. Please use iFrame code and then try to replicate the issue again after embedding with iFrame.
Or you can use JotForm application for Weebly. I think that this option may be the first way for you. Please look at the guides and article below:
