Why can't I see questions on submitted forms that people are required to fill out?

    We have one question on our form which is : How have you heard about us? 

    It is ticked as required and shows normally on the form.

     

    But as soon as people submit the form and I view the submission it does not appear in the completed form.

     

    Please help.

    Hello Josie,

    I'm sorry for the trouble caused to you. 

    Do you mean to say that this question is missing from the submission email you receive? Please note that if you edit your email alert template, it becomes dirty. That means any changes you make in your form will not reflect in email alert. The best option to solve this problem is to delete the email alert and add a new one. A new email alert will always include all the fields of your form. The following guides should help you:

    Add new email alert:  https://www.jotform.com/help/25-Setting-Up-Email-Notifications

    I did check your form's submission page and it does display the answers without any problem. Please check the screenshot below:

    Thanks for the quick reply.
    You are right! I can see the answer to this question on the jotform
    submission website.
    But unfortunately I can't see this question when I download the form as PDF
    ...

    Unfortunately, your screenshot didn't make it to the forum, can you please try re-attaching it by following this link.

    I did some testing, and I found from the submission example that you're having an issue with this form. So I checked the submissions, and you're right, when you download PDF, you can't see the "how did you head about us" field. But this is because this field is not added to the PDF layout, when I opened "Customize PDF", I could see that this field is not added to the layout. Please see:

    You can simply fix this by adding this field to your PDF layout, please see the guide below for instructions on how to customize your PDF report and add/remove fields:

    Hi there,
    I have tried to modify the PDF form. I can see the question I wanted now
    but everything is out of alignment. Most of the questions don't match the
    answers anymore.

    Hello CompleteCityHealth,

    If most of the questions doesn't match the answers that means you have changed the question alignment in submission PDF template. I would suggest you to please edit your Submission PDF template and reset it to the default template. Please check the screenshot below on how to reset submission PDF template:
