In isolated instances a form is filled out and it is not forwareded to me . This just happened yesterday on a form that was filled out and not forwarded and only a few times last year --

Are you referring to the email notifications? I checked your form and I can see that the notification is configured correctly. The recipients are also not on the bounce list.
I checked the email history logs and I can see that the emails were sent successfully.
I suggest you check the Spam folder or Junk mail. If the issue persists, then I suggest you use an SMTP as the Sender Email. Here's how: How-to-Setup-SMTP-for-a-Form.
You may also try creating a separate email notification for the other recipient. It means that you will have 2 notifications. Here's a guide: Setting-up-Email-Notifications.
