Create a form with different URLs.

I want to create a standard evaluation Jotform and send it to multiple clients using multiple links for the same form. All user information would returns to the same form.

Thanks for reaching out to Jotform Support. You can create a custom URL for your form, but you can't create multiple custom URLs for one form at once. If you want, you can create a custom URL for one client, and once that client is done submitting his response, you can then change the form's custom URL for the next client. Let me show you how to create one:

1. In Form Builder, click on Publish in the orange navigation bar at the top part of the page.
2. Under Quick Share, hover your mouse over the form URL, and click on the pencil icon on the right end of the URL field.
3. Customize the URL as allowed, and click on Apply.
4. Click on the Copy Link button to copy the custom URL.

If this doesn't work for you, you can also clone your form, so you can have different URLs, then connect the cloned forms within your account in Jotform Tables, which allows you to manage submission data from multiple forms in one place. Let me show you how to get data from other forms in Jotform Tables:

1. Go to Jotform Tables, and click on Add Tab in the upper-left corner.

2. In the dialog that pops up, select Tables (or choose your preferred tab style).

3. Select Connect to Form.

4. Then, choose a form to connect.

5. Choose the columns to include in the tab, and click on Create.

A new tab should appear where you can view and manage the connected forms' data. Note that changes to the entries will reflect in the connected form’s records.
