By default, Jotform forms use a unique form ID in their URL. If you’d like to make your form link more personalized, professional, or easier to remember, you can create a custom form URL using your text instead of the generated ID.
A custom URL can make your form easier to share with respondents and help reinforce your branding. For example, you can use your form name, event title, campaign name, or another meaningful phrase as part of the link, creating a cleaner and more recognizable URL for your audience.
Notes
- If you change your form’s custom slug, the previous custom URL will no longer work, so be sure to update any links you’ve already shared.
- Changing your Jotform username will affect all custom form URLs that use your old username, making those links unavailable.
- Your form’s original Jotform URL will continue to work even if you change the custom slug or your username.
The custom form URL has the following structure:

To make your form URL more personalized and easier to remember, just follow these steps:
- In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
- Then, click on the Pencil icon next to your form URL.
- Now, enter your desired URL slug after https://form.jotform.com/yourusername/
- Once you’re done, click on Apply.
That’s it! You can now share your form using its custom URL, making it easier for respondents to recognize and remember. If you ever want to remove the custom URL and return to the default one, simply click the Circular Arrow icon under Share With Link.

I run a not for profit Psychotherapy Clinic and my IT staff just told me you cost €5000 just for a custom domain and to remove the white label thing. That is worth more than the conference i just held. Is there a way that I can simplfy this without breaking that bank. The price is really uncalled for.

How do I change the URL path to a folder/? I am not able to remove the owner's name in the path. It really defeats the purpose if we can't categorize these forms under common folder irrespective of who develops. I don't want to use another proxy/redirect/vanity url approach to achieve this. Hoping that this has already been addressed. I am pretty new...

Can you activate a custom URL again if it was accidently removed? For some reason I think the custom URL was removed on my link. I published this custom URL everywhere and if it cannot be retrieved I would have to change it everywhere. And if I can use the custom URL again, will the URL with the number still work? Because I started to use this URL with the number now.
