How to Create a Custom URL for Your Form

You can choose to use a custom slug, instead of the form ID, for your form’s URL. This is useful if you want to publish your form with a customized or easy-to-remember link. For example, you can add your own form name at the end of the URL along with your Jotform username.

The custom form URL has the following structure:

https://form.jotform.com/{yourUsername}/{your-custom-url-slug}

To give your form a custom URL

  1. In Form Builder, click on Publish at the top.
  2. Next, click on the pencil icon next to your form’s link, under Link to Share.
  3. Type in your desired URL slug then click on Apply.

You can now share your form using the new URL. To delete the custom URL, click on the reset button under Link to Share.

Notes:

  • Changing the form’s slug invalidates its old custom URL.
  • Changing your username invalidates all custom form URLs with your old username.
  • The original form URL works regardless of slug or username changes.
  • Custom form domains are available in Jotform Enterprise only.
