Could you kindly clarify your question?

If you are looking at creating PDF forms, it is possible using the Fillable PDF form creator. You can check our guide below for further details:

    How-to-Create-a-PDF-Form

If are looking at showing texts/fields on your form based on user's answer, you can achieve this by using conditional logic. Please check the guides below for further details:

    Smart-Forms-Using-Conditional-Logic

    How-to-Show-or-Hide-Fields-Base-on-User-s-Answer

Once you have these conditions added to your forms, sent the form to your users for completion and the form gets submitted; the submissions can be downloaded as PDF documents / sent as attachments in email.

    1. To attach PDF documents in emails sent to you after your form gets submitted, refer to:

    How-to-include-submissions-as-PDF-attachment-in-the-email-notifications

    2. To attach PDF documents in emails sent to your users once they have submitted your form, refer to:

Do let us know if this is not what you were looking for and we will be happy to provide any further assistance.

I understand what you are looking for. If the user enters the second choice in the first question, a text field (entry) shows up and you want this entry to be shown in the PDF.

What you will need to do is, open your form from your My Form page and click on Submissions.

In the Submission page, you will have to select the "gear" icon as shown below and enable "Show Headers and Texts". By default, this option will be unchecked. You will have to manually check this option. Then you will be able to view the advice/text field in the PDF. Please refer to the screenshot below.

If you do not want to show all headers and texts, you can customize the PDF document. Just select the"Customize PDF" option as shown below in your Submission page.

This will take you to the PDF Editor. At the initial instance, you will be prompted to provide a name and select a layout for your PDF document. Once done, select "Create".

Once this is done, you will enter the PDF Editor page, where you can delete fields that you do not want to be shown on the PDF. You can then download the PDF/ preview the same.
