Questions about HIPAA compliant patient forms.

I need help getting started with HIPAA compliant patient intake forms

In particular, I'd like an ovierview of the form workflow process. I presume patients can complete forms on our website, or by email, or in the office on a tablet.  How are the completed forms then received by the practice? What do medical offices then do with the forms? Are they manually uploaded as EMR into a practice management software?

Any chance the data can be directly integrated into our PMS? Or can the forms themselves be somehow automatically uploaded?

    Thank you!

I presume patients can complete forms on our website, or by email, or in the office on a tablet. — Patients can complete the form on your website, email or office tablet. As long as you have internet connection and can view the form URL or website, your patients can complete the form.

How are the completed forms then received by the practice? — The completed forms are received via email notification or auto response. Please check our guide below.

What do medical offices then do with the forms? Are they manually uploaded as EMR into a practice management software? — They can view your form in Jotform and if they need the data, they can access it via submission page https://www.jotform.com/help/269-How-to-View-Form-Submissions.

Any chance the data can be directly integrated into our PMS? Or can the forms themselves be somehow automatically uploaded? — Check what integration apps is available in Jotform via this link https://apps.jotform.com/.
